What Is Profhilo®? IBSA’s Biostimulatory Skin Remodeller Explained

PROFHILO® is manufactured by IBSA Farmaceutici Italia and classified as an injectable medical device rather than a dermal filler. The original PROFHILO® H+L presentation carries 32 mg of high-molecular-weight hyaluronic acid (H-HA) and 32 mg of low-molecular-weight hyaluronic acid (L-HA) in a single 2 mL syringe, giving a total of 64 mg of HA and a concentration of 32 mg/mL.

Mechanistically, the H-HA fraction binds water, supports the extracellular matrix, and releases slowly over time. The L-HA fraction interacts more directly with fibroblasts and keratinocytes, driving the intracellular signaling associated with tissue remodeling.

Laboratory work on hyaluronan hybrid cooperative complexes has shown upregulated expression of several collagen types, along with elastin, across fibroblast, keratinocyte, and 3D reconstructed skin models, consistent with the collagen and elastin production changes clinicians see weeks later.[1] This mechanism is the reason PROFHILO® is positioned as an injectable skin treatment for tone, texture, laxity, and hydration rather than for contour or projection.

NAHYCO® Hybrid Technology — What Makes PROFHILO® Different

NAHYCO® is short for Hybrid Cooperative Complexes, and this technology distinguishes PROFHILO® from both classic cross-linked HA fillers and standard non-cross-linked biorevitalizers. Rather than using BDDE or another chemical cross-linker to stabilize the HA chains, IBSA thermally stabilizes H-HA and L-HA together into cooperative complexes that remain stable, distribute broadly through tissue, and release progressively.

For practitioners weighing PROFHILO® against chemically cross-linked products, the practical implications are:

  • No BDDE in the product, which is associated with a lower risk of delayed hypersensitivity reactions than chemically cross-linked HA formulations
  • Ultra-pure biofermentation-derived HA with endotoxin content well within pharmaceutical thresholds
  • Dual-fraction behavior in tissue where sustained biostimulation and earlier hydration effects are both attributable to the same syringe rather than layered from separate products.

This combination is what makes NAHYCO® central to how PROFHILO® is described as a skin rejuvenation and skin quality treatment, since the goal is to improve volume and elasticity biologically rather than mechanically.[1]

PROFHILO® Product Range — H+L, BODY KIT, and STRUCTURA

All three PROFHILO® lines share NAHYCO® technology, but they differ in HA content per syringe, target tissue, and injection technique.

PROFHILO® H+L

The original 2 mL, 64 mg presentation. It is the reference PROFHILO® skin booster for the face, neck, and hands, and it is the formulation validated with the five-point BAP protocol described further down this page. Most published clinical data and PROFHILO® reviews are about this specific product.

PROFHILO® BODY KIT

Designed for larger surface areas where laxity, dehydration, and quality decline respond poorly to a facial protocol. Each 3 mL syringe contains 96 mg of HA, and the kit targets the abdomen, inner arms, inner thighs, and knees. Depending on presentation, it may also include patches and topical support material for the post-treatment window. Its use has expanded steadily as non-surgical body skin quality has moved further into aesthetic practice.

PROFHILO® STRUCTURA

Each 2 mL syringe contains 90 mg of HA, and the product is intended for placement into the superficial fat compartments of the lateral cheek, typically through a cannula from a defined entry point, to restore adipose tissue homeostasis and provide mild structural support alongside skin remodeling. STRUCTURA should not be confused with a conventional cross-linked cheek filler, nor should it be injected using the standard BAP technique developed for H+L.[4]

Is PROFHILO® a Filler?

PROFHILO® H+L is a biostimulatory skin remodeller, not a volumizing filler. The formulation does not contain cross-linked HA, and it is not intended to correct contour or project features the way JUVÉDERM®, Restylane®, or comparable dermal fillers do.

PROFHILO® STRUCTURA is the nuance in that answer. It provides mild structural support in the lateral cheek, but its role is closer to adipose tissue restoration than conventional cheek augmentation, and its regulatory positioning still sits within the skin-remodeling category rather than the volumizing category. The clinical purpose of the PROFHILO® range as a whole remains skin quality, elasticity, and laxity, not the correction of static volume loss.

PROFHILO® Injection Points — The BAP Technique

The BAP (Bio Aesthetic Points) protocol was developed alongside PROFHILO® H+L, and the two are closely tied. The technique specifies five anatomically defined PROFHILO® injection points per side of the face, with a 0.2 mL bolus at each point, for a total volume of 2 mL and 64 mg of HA per session.

The five points on each side sit around:

  • The zygomatic (upper cheek) region
  • The nasal base
  • The tragal or preauricular region
  • The chin
  • The mandibular angle

These locations were selected to allow the formulation to distribute through tissue while avoiding the face’s higher-risk vascular corridors. The distributional behavior of NAHYCO® hybrid HA is why so few points are needed: the product spreads on its own rather than remaining as a fixed bolus, so a dense mesotherapy-style grid would be redundant. A small observational study evaluating BAP with H+L at four-week intervals reported significant improvement in hydration and elasticity measurements alongside generally good tolerability.[5]

BAP is a formulation-specific injection technique. It is not interchangeable with cocktail-style mesotherapy, and it is not appropriate for PROFHILO® BODY KIT or PROFHILO® STRUCTURA, both of which use their own point maps and delivery approaches. Individual anatomy still varies, and vascular assessment, depth, angle, and volume decisions belong with the injector.

Treatment Areas — Where Is PROFHILO® Used?

PROFHILO® H+L is used clinically across the mid- and lower face (cheeks, perioral skin, lower face), the neck, the décolletage, and the dorsal hands. These areas are sometimes loosely grouped as cheeks, eyes, feet, forehead, and hands, but not every one of those areas is an on-label or clinically routine PROFHILO® target. The lips, in particular, are not treated as a PROFHILO® region in the way a lip filler area is, and direct periocular injection with H+L is off-label and requires careful judgment rather than a default extension of the facial protocol.

Broader areas such as lips, lower face, neck, and décolletage are treated as part of the facial and cervical protocol only where individual anatomy and product movement support it.

PROFHILO® BODY KIT is used on the abdomen, inner upper arms, inner thighs, and knees. It uses its own body treatment-point map rather than the five-point facial BAP grid, and it is generally selected for skin laxity and quality rather than for genuine volume restoration.

PROFHILO® STRUCTURA is used primarily in the lateral cheek and superficial fat compartments of the mid-face, with limited application in the neck depending on injector judgment.[4] It should not be marketed or administered as facial skinboosters that are interchangeable with H+L.

PROFHILO® Treatment Effects Timeline

  • After 1 week: Mild swelling, redness, tenderness, bruising, or small raised areas usually settle. Some patients may notice early hydration or glow, but this is not the final result.
  • Around 4 weeks: Improvements in hydration, texture, and elasticity become more noticeable. The second session is also commonly scheduled at this stage.[3]
  • After 8 to 12 weeks: Fuller PROFHILO® results are usually assessed after the second treatment. Before-and-after photos may show improvements in skin turgor, elasticity, texture, hydration, and mild laxity.[2]

Many practitioners reassess at around six months and may recommend one or two maintenance sessions each year. Results vary according to age, skin condition, lifestyle, and treatment area.

Mesotherapy vs Profhilo®: Key Differences

Mesotherapy generally involves 20 to 50 or more superficial micro-injections of a cocktail containing non-cross-linked HA, amino acids, vitamins, minerals, and often antioxidants. Its clinical rationale is nutritional and hydrational, delivered across a dense injection grid.

PROFHILO® runs through five BAP injection points per side of the face and one syringe carrying 64 mg of high-purity HA. Its rationale is biostimulatory, and the collagen and elastin production response is expected to be more pronounced and longer-lasting than what a mesotherapy protocol produces.

Regulatory Status

PROFHILO® and PROFHILO® BODY are CE-marked medical devices, with the original PROFHILO® first CE certified in 2015 and PROFHILO® BODY following in 2020.[3] Registration status, prescription requirements, and permitted commercial channels differ by country, and practitioners should confirm what applies in their jurisdiction before ordering.

PROFHILO® is not FDA-approved for aesthetic skin remodeling in the United States. On July 8, 2026, Evolus announced an agreement with IBSA to develop and commercialize PROFHILO® in the US, with Evolus taking on the clinical and regulatory work needed for a future FDA submission. That agreement indicates US approval is a planned milestone rather than a current authorization, and US-based practitioners should not treat European CE marking or online availability as equivalent to US clearance.

PROFHILO® Compared With Other Biostimulatory Injectables

NUCLEOFILL™

NUCLEOFILL™ is built on polydeoxyribonucleotide (PDRN) technology derived from purified fish DNA, and its mechanism relies on A2A adenosine receptor activation to prompt fibroblast activity and tissue repair. PROFHILO® acts through HA-mediated biostimulation without any nucleotide component. Where NUCLEOFILL™ carries a dedicated periocular option (NUCLEOFILL™ Medium), PROFHILO® H+L is used around the eye area only off-label at the injector’s discretion. Neither product is a filler, but PROFHILO® STRUCTURA adds a structural component that the NUCLEOFILL™ range does not offer.

SUNEKOS®

SUNEKOS® combines non-cross-linked HA with a patented amino acid complex (glycine, L-proline, L-leucine, L-lysine, L-valine) intended to supply the building blocks for collagen and elastin synthesis at the extracellular matrix level. PROFHILO® has no amino acid component and works purely through hybrid HA biostimulation. SUNEKOS® has periorbital and body variants (200, 1200, PERFORMA), while PROFHILO® covers those broader indications through BODY KIT and STRUCTURA. Both are CE-marked; neither is a traditional HA filler, and clinics often carry both to cover different treatment rationales.

JALUPRO®

Unlike PROFHILO® (manufactured by IBSA), JALUPRO® is produced by Professional Derma SA and focuses on Amino Acid Replacement Therapy (AART) combined with hyaluronic acid. JALUPRO® Classic and JALUPRO® HMW combine non-cross-linked HA with a smaller amino acid mixture and sit primarily in the biorevitalizer category, focused on hydration and superficial skin quality. PROFHILO® delivers a substantially higher HA load per session (64 mg per 2 mL H+L) and a defined biostimulatory mechanism. JALUPRO® HMW is the closest JALUPRO® formulation to PROFHILO® H+L, though even there the compositions and indications diverge.

REJURAN®

REJURAN® is South Korean in origin, and its mechanism is PDRN-based rather than HA-based. Its lineup includes REJURAN® HEALER for standard facial rejuvenation, REJURAN® is specifically indicated for the periorbital area, and combination formulations. Practitioners choosing between the two often do so along the mechanism axis: PDRN biosignaling for tissue repair with REJURAN®, HA-fibroblast biostimulation for skin quality and laxity with Profhilo®. In our experience, clients frequently stock both, since patient preference for a hyaluronic-acid-only product versus a polynucleotide-based product varies.

VISCODERM®

VISCODERM® is also an IBSA product, and it sits closer to hydration and biorevitalization than to biostimulation. VISCODERM® HYDROBOOSTER adds a micro-lifting element with a still-modest HA load. PROFHILO® contains substantially more HA per session, distributes more diffusely, and carries a clearer biostimulatory clinical profile.

Clinical Benefits of PROFHILO® for Aesthetic Practitioners

  • 64 mg of HA per 2 mL H+L syringe, one of the highest per-session HA loads among biostimulators
  • Hybrid H-HA and L-HA formulation via thermal stabilization, with no BDDE cross-linker
  • A defined five-point BAP protocol per side of the face, faster to deliver than a mesotherapy grid
  • Wide product coverage across face, body, and adipose-restoration indications through H+L, BODY KIT, and STRUCTURA
  • Published clinical data supporting improvement in hydration, elasticity, tone, and skin quality[2]
  • A tissue-diffusion behavior that lets fewer points cover broader tissue

However, several individual clinical studies remain small, and there is not yet an extensive body of head-to-head comparison against other biostimulators or fillers. Product authenticity, LOT traceability, storage handling, and injector training carry as much weight as any headline HA figure. Doctor Medica’s staff can advise on documentation, sourcing, and local regulatory considerations before you commit to an order.

PROFHILO® Aftercare

Patients are typically advised to:

  • Avoid touching or pressing the injection points for about six hours
  • Keep the treated area clean
  • Skip strenuous exercise for 24 hours
  • Avoid alcohol on treatment day
  • Stay away from saunas, steam rooms, and intense heat for at least 24 hours
  • Hold off on makeup until the injection sites have closed
  • Delay facial massage or other injectable procedures until the area has settled
  • Protect the area from strong UV exposure

IBSA’s safety documentation specifically recommends avoiding UV exposure and using high-protection sunscreen for several days post-treatment.[3] Small bumps at the BAP points are normal after PROFHILO® injections and usually flatten within 24–48 hours as the product distributes. Persistent swelling, increasing pain, heat, blanching, discoloration, or a growing nodule at the one-week mark isn’t normal aftercare and should be assessed promptly.

Side Effects and Safety

Common side effects of PROFHILO® are consistent with any injectable HA treatment and are largely tied to the local injection response: redness, swelling, tenderness, bruising, itching, warmth, or transient papules at the BAP points. A recent systematic review reported that adverse effects were generally mild and often resolved within about 72 hours.[2] Post-marketing surveillance is consistent with the same profile, with edema, erythema, and discomfort among the most commonly reported events.

The absence of BDDE cross-linker is a genuine advantage against chemically cross-linked HA products in terms of delayed hypersensitivity risk, but it does not eliminate rare complications. Vascular compromise, infection, prolonged inflammation, nodule formation, and hypersensitivity remain possible with any HA injection. PROFHILO® should never be injected into a vessel, and IBSA’s safety guidance advises against use during pregnancy or breastfeeding, in patients with autoimmune disease, over active inflammation or infected skin, or in patients with known component hypersensitivity.[3] PROFHILO® and PROFHILO® BODY should not be combined with medium-to-deep peeling or laser resurfacing in the same treatment area during the same session.

Always review individual patient history, medications, active skin conditions, and the planned injection plane before treatment.

Frequently Asked Questions

What is Profhilo®?

PROFHILO® is an injectable medical device developed by IBSA Farmaceutici Italia. It uses patented NAHYCO® Hybrid Technology to combine 32 mg of high-molecular-weight HA and 32 mg of low-molecular-weight HA in a 2 mL syringe, for 64 mg of HA per session. It is designed to remodel skin and biostimulate collagen and elastin production rather than to add contour or projection.

Is PROFHILO® a filler?

No. PROFHILO® is classified as a biostimulatory skin remodeller, not a dermal filler. PROFHILO® H+L is used to improve skin quality, elasticity, and laxity through HA-mediated fibroblast biostimulation, and it does not contain cross-linked HA. PROFHILO® STRUCTURA introduces a mild structural role, but the range still sits outside the volumizing filler category.

What are the PROFHILO® injection points?

PROFHILO® H+L is administered through the Bio Aesthetic Points, or BAP, technique: five anatomically defined injection sites per side of the face, positioned to allow tissue-wide distribution while avoiding high-risk vascular structures. Sites usually sit around the zygomatic region, nasal base, tragal area, chin, and mandibular angle. The protocol is distinct from mesotherapy grids and is specific to H+L.

What is the PROFHILO® treatment effects timeline?

Early hydration and glow can appear within the first few weeks after session one, while the second session is typically scheduled around four to six weeks later. Fuller effects on texture, elasticity, and skin quality generally develop across the eight-to-twelve-week window after the second session, when the biostimulatory response has had time to mature. Individual response depends on age, baseline skin condition, and treatment plan.

What is the difference between PROFHILO® and mesotherapy?

Mesotherapy delivers a cocktail of HA, vitamins, amino acids, and other ingredients through many superficial micro-injections and works primarily through nutritional and hydration delivery. PROFHILO® runs through five BAP points per side and works through HA-mediated biostimulation, with a much higher HA load per session. Both improve skin quality in different ways, and many clinics offer both.

How does PROFHILO® compare with Sculptra®?

Sculptra® uses poly-L-lactic acid to prompt gradual collagen production, with results that develop over months and can last two years or more. PROFHILO® uses hybrid HA to biostimulate collagen and elastin via a different biological pathway, with earlier-appearing effects and a shorter maintenance interval. Sculptra® tends to suit patients seeking longer-lasting collagen induction, while PROFHILO® suits patients focused on hydration, elasticity, and overall skin quality.

What is PROFHILO® aftercare?

Avoid touching or pressing the injection points for around six hours, skip strenuous exercise, alcohol, and heat exposure for 24 hours, and avoid facials or overlapping injectables in the treatment area for approximately two weeks. Mild swelling, papules, and redness at BAP points are common and usually settle within one to two days.

Where can practitioners buy PROFHILO® wholesale?

Licensed practitioners can buy PROFHILO® wholesale through Doctor Medica, with pricing that scales to order volume and access to IBSA products across the full range. Clinics should confirm product authenticity, LOT traceability, expiry information, and local regulatory permissions before placing an order, and Doctor Medica’s staff can advise on documentation and shipping to your market.
