ORENCIA® is a selective immunosuppressive agent containing abatacept, a biologic fusion protein that modulates T-cell activation. It is used in the treatment of moderate to severe rheumatoid arthritis (RA) in adults and polyarticular juvenile idiopathic arthritis (pJIA) in children aged 6 years and older. It is particularly effective in patients who have had an inadequate response to traditional disease-modifying antirheumatic drugs (DMARDs), including methotrexate or TNF-alpha inhibitors.
ORENCIA helps reduce joint inflammation, slow disease progression, and improve physical function. It is available as a 250 mg lyophilized powder for intravenous infusion and must be reconstituted and diluted before administration under medical supervision.
ORENCIA® is administered by intravenous infusion under healthcare supervision.
Key features:
- Dose based on body weight
- Infused over 30 minutes every 4 weeks after loading doses
Administration protocol:
- <60 kg: 500 mg (2 vials)
- 60–100 kg: 750 mg (3 vials)
- >100 kg: 1 g (4 vials)
- Initial doses at week 0, 2, and 4, followed by every 4 weeks
- Reconstituted in 10 mL sterile water per vial and diluted to 100 mL with 0.9% sodium chloride
- Use a 0.2–1.2 μm low-protein-binding filter during infusion
Indications:
- Moderate to severe rheumatoid arthritis (RA) in adults
- Active polyarticular juvenile idiopathic arthritis (pJIA) in patients aged ≥6 years
- Used in combination with methotrexate
Contraindications:
- Hypersensitivity to abatacept or excipients
- Severe or uncontrolled infections
- Use with other biologic agents like TNF inhibitors, anakinra, or rituximab
Possible side effects:
- Common: Upper respiratory infections, headache, nausea
- Serious: Severe infections, malignancies, infusion reactions, hypersensitivity
- Pediatric: similar to adults, with fever and ear infections also observed
